| Welcome Heart Failure Champions to “The Bridge Project Online”. These continuing education modules are designed for your team to close the gap between evidence and practice in your skilled nursing and long term care facility. They provide information to help improve the quality of heart failure care. |
HF management programs can improve quality of life and function using a plan which combines staff education, commitment, and an interdisciplinary team. We believe we can reduce the number of hospitalizations which lead to confusion, physical function decline, weakness and falls through this program. These modules include topics such as better understanding of heart failure, careful assessment, following simple standards of care, dietary and activity management. |
| The system is designed your busy schedule in mind. If you don’t have time to complete the module in one sitting, you can log off and the system will take you back to the place you left the next time you log on. You can also choose to start over if you wish. |
