In spite of evidence-based guidelines and multiple randomized clinical trials to guide care, heart failure (HF) hospitalization costs are high, often due to poor after discharge follow-up, high rates of 30-day readmission, inadequate prescription and titration of medications, limited patient education for self care and poor communication between providers. The N-HeFT Program gives qualified medical care teams an opportunity to meet with heart failure experts to share clinical challenges in the management of heart failure. The program is approved for CME and CEU.  The allied health program is designed to meet the unique needs of medical teams in home health, skilled facilities, nursing homes, palliative care, hospice or other areas related to the care of heart failure patients and their families.  The education provides optimal approaches for inpatient or outpatient care, blending highly interactive lecture, case discussion, observation, and hands-on participation specifically geared to the needs of allied health professionals. N-HeFT faculty can also help you design your own chronic care program for your heart failure patients.

Typical teams may consist of physicians, nurse practitioners, nurses, nursing assistants, physician assistants, pharmacists, administrators, social workers and other health care professionals involved in the management of heart failure patients in a variety of settings.  You will have the opportunity to participate in training specifically designed for your organization and visit a heart failure center of your choice if you wish.  Your team will set goals for improvement and also receive supplemental resources such as quality tools and patient education materials to improve both clinical and economic outcomes. Following the training, you will have an opportunity for three follow up consultations with host site faculty.

The explosion of new information in the field of cardiovascular disease and the increasing demand for accountability necessitates better ways to communicate real time information.  Online curriculum provides the format for introducing new concepts, reinforcing complex directives, and enrichment.  We offer core curriculum, electives and presentations of interest to the medical professional involved in the general care of patients.  You may also learn what national experts have to say about cardiovascular research and women related to heart failure or coronary artery disease. 

In addition to expanding the scope of the program and adding depth, flexibility and convenience, the online component of N-HeFT includes a bridge to our network of experts through our email.
